Before proceeding to the appointment calendar to book an administration appointment, please ensure:

• You a currently registered in the pharmacotherapy program with your prescriber, and they have an active permit for your program

​

• You have already had an initial consultation with your prescriber

​

• Your prescriber has already provided a prescription for LAIB to the pharmacy

​

• You have valid ID such as photo ID, Medicare card and concession card if applicable

​

• You are well enough to receive the injection
